The present document sets forth conditions for obtaining authorisation to perform health risk assessments.

Conditions for granting a licence
The following persons shall be entitled to be awarded the authorisation:
- individuals (natural entities) over 23 years of age
- persons of probity – individuals who have not been found guilty of any deliberate criminal offence or criminal negligence associated with activities to assess various aspects of living and working environments and their impact on public health
- professionally eligible persons, where professional eligibility shall be demonstrated by:
- documentary proof of completed university education in the field of medicine or natural sciences, and/or certificate attesting to successful completion of a training course in health risk assessment undertaken at a state organisational unit or with a semi-budgetary organisation authorised by the Ministry of Health, where the individual concerned took part in the course with a view to obtaining professional knowledge, and at least
- five years of experience in the field of health and environmental protection -Experience in the field of health and environmental protection is regarded as any occupational activity involving assessment of aspects of the living and working environment and its impact upon human health, based on confirmation from the employer or results of such assessments as submitted. The length of experience shall not include basic, substitute or civilian service, or time spent on maternity or parental leave. The Ministry of Health shall rule on whether or not to recognise work experience that was interrupted for a period of at least 5 years at the request of an individual. When assessing proof of experience issued by the competent body of another EU Member State, the procedure stipulated under a special legal regulation shall be employed.
- submit a solemn declaration to the effect that the individual has no financial or other interests that may have any effect on results when assessing health risks
A further condition is the passing of a professional eligibility exam. In the exam, the individual concerned must demonstrate knowledge specified by the relevant implementing legal regulation. The implementing legal regulation further regulates the composition of the commission and conditions applying to the professional eligibility exam.
Application requirements
The application shall contain:
- name
- surname
- highest completed education and the field of expertise, academic degrees
- permanent residence or contact address, where the applicant is not a resident in the territory of the Czech Republic
Annexes:
- Documentary proof in support of the above shall be annexed to the application (see the conditions)
Requirements connected to the provision of services by a provider from a different Member State
- No authorisation certificate is required for individuals established in another EU Member State if they can prove that they are
- a national of an EU Member State,
- a holder of authorisation to perform activities indicated under paragraph 1 pursuant to the legal regulations of another EU Member State should such an authorisation be required in the State concerned,
- an individual of probity.
- For carrying out assessments of health risks, the individual concerned must submit all previous health risk assessments to the Ministry of Health.

Procedure for granting a licence procedure
- The applicant shall first pass a professional eligibility exam. The application to sit the exam is an application to be awarded a certificate of authorisation and must be sent by the applicant to the State Health Institute.

Validity period of licence
- 2 years, may be renewed for 5 years
