The article provides basic information on Single Registration Forms designed both for legal and natural persons in order to simplify the process of starting up and conducting a business in the Czech Republic. Forms can be used for example for trade notification, licence application, or tax registration (VAT, road tax, real estate tax, etc.). Available are forms, instructions and annexes.
Natural persons
The Single Registration Form (SRF) replaces different forms entrepreneurs had to fill in before starting up and even when conducting the business at trade licensing offices, revenue authorities, competent social security administration, employment offices, and health insurance companies. Requirements of these authorities have been integrated into a single form which is more entrepreneur-friendly and makes the administration for entrepreneurs easier. Natural persons may use the form, for example, for trade notification, application for licence, tax registration (VAT, road tax, real estate tax, etc.) or notification of commencing self-employment.
There are usually two possibilities to fill in forms. To fill forms on-line via BusinessInfo.cz portal or manually fill in the printed form in PDF format. Instructions are also available for some forms.

Downloadable application for filling in SRF forms
The Single Registration Form (SRF) may be submitted either in a paper or in an electronic way. Both ways are equal, the electronic way makes it more comfortable to enrepreneurs who can send it directly to the electronic register of the Register of Trade Business or to a trade license office via data boxes. The tool for generating electronic submission (EPO) in a format of the Single Registration form has been available to the entrepreneurial public since 1. 10. 2008.
The electronic form may be created through the SRF application which can be dowloaded free of charge:
The instructions for installation are in Czech only however, the installation of the SRF is available also in English. The form itself is in the Czech language only but we provide you English translations of instructions both for legal and natural persons.
If you have any questions regarding filling–in the SRF, it is suggested to contact the nearest trade license office where its employees will help you. If you have any problems with installation, start up or operation of the SRF application, you can contact the technical support of the SRF application by e–mail: jrf@i.cz.
Sending the filled-in submission
After downloading and successful installation of the SRF on your computer, you can generate the EPO and submit it, according to the instructions, directly to the electronic registry of the Register of Trade Business or by means of IS data boxes to the data box of the relevant trade license office.
Register of Trade Business
An approved electronic signature is required. Qualified certifications of the following licensed certification authorities may only be used (alphabetically ordered, as of 21. 5. 2010): Česká pošta, s. p.; eIdentity a. s.; První certifikační autorita, a. s.
The up-dated list of qualified providers of certification services is available on the website of the Ministry of Interior of the ČR.
Data box of the relevant trade license office
A person sending the submission is obliged to own an activated data box. The relevant trade license office designated to handle the EPO processes it in the Register of Trade Business according to the law and settles it in an electronic way if possible.
Finding the status of the electronic submission
The following description applies solely to submissions sent to the electronic registry of the Register of Trade Business. The submission may be searched by the number of the submission which was allotted by the Register in the "Ennnn" or only the number "nnnnn" formats (i. e. for example "E1111" or "1111"). This number is written in the head of the PDF form printed after sending the submission.
- Finding the state of the electronic submission(in Czech only)
The status may be the following:
- not found – the given submission number was not found,
- being delivered to the Trade License Office (TLO) – the submission was sent, it is being delivered to the TLO,
- delivered to the Trade License Office – the submission was delivered to the Trade Register system of the relevant trade license office, its handling has not started yet,
- received by the Trade License Office – the TLO received the submission and it is being processed,
- settled – the submission was processed by the Trade License Office.
If the submission was successfully delivered to the Trade License Office data box, an electronic advice of delivery is sent to the data box of the submitter.
Additional Information
Downloading of installation files, installation itself and running the SRF application proceed through the SSL protocol so a warning icon or a webpage with „Security warning“ may appear – it means that Česká pošta certificates PostSignum QCA (PostSignum Root QCA, PostSignum Root QCA 2) are not installed. The website CA Postsignum describes steps necessary to take in order to install the certificates. An icon requiring confirmation of the trust of the server certificate may appear after the installation is finished if the user’s system is set to trust certificates issued by the PostSignum Certification authority of Česká pošta.
Only a SHA-2 hash function may be used to generate electronic signatures of documents since 1. 1. 2011. Instructions for setting up an electronic signature in the SRF application – SHA-2.
